Below are links to forms you may have to complete and submit to the office for various circumstances.
- TK thru 8 Enrollment Application 2025-2026 – Please fill out this form for application to the elementary school, grades TK – 8.
- Preschool Enrollment Application 2025-2026 – Please complete this form to apply to preschool.
- Sports or Youth Activity/Service Permission Form – Please fill out this form if your child will be participating in a school sport or school-sponsored athletic activity.
- Diabetic Consent Form – Please fill out and turn it in to the office if your child is diabetic and may need to administer blood sugar testing or related testing during school.
- Medication Authorization and Permission Form – Please fill out this form if your child requires any special medications to be administered or needs to use inhalers due to asthma.
- Parental Permission For Internet Access – Please fill out this form to authorize internet access during class time for educational research and learning activities such as IXL.
