- K thru 8 Enrollment Application 2020-2021 – Please fill out this form for application to the elementary school, grades K – 8.
- Preschool Enrollment Application 2020-2021 – Please fill out this form for application to preschool.
- Sports or Youth Activity/Service Permission Form – Please fill out this form if your child will be participating in a school sport or school sponsored athletic activity.
- Diabetic Consent Form – Please fill out and turn into the office if your child is diabetic and may need to administer blood sugar testing or related testing during school.
- Medication Authorization and Permission Form – Please fill out this form if your child requires any special medications to be administered or needs to use inhalers due to asthma.
- Parental Permission For Access To The Internet – Please fill out this form for authorizing access to internet during class time for educational research and learning activities like IXL etc.
